All Graduate Teaching Assistants must have a performance
evaluation. This should be completed by April 7.
For GTAs who may be applying for continuing support in the Summer
or Fall
2024 semester, this evaluation is a critical component in
determining whether you would be rehired.

Graduate students: Make an appointment to meet with your faculty
supervisor for this performance review. If you were assigned to
more than one course, you must do this for each course.

Faculty: in preparation for that appointment, please
download this
form and fill it out (all pages).
- The form is designed to be completed in Adobe
Reader or Acrobat or similar tools.
- Do not fill it out in your web browser's PDF preview
tool. You will not be able to save your work.
- Faculty: Discuss your evaluation with your
TA in your meeting.

Faculty: Sign the form (see below) and email the signed file to the graduate student.

Graduate students: Check one of the boxes at the bottom of the
first page,
then sign the form (see below).

Graduate students: Submit your form as follows:
- Via SharePoint
- Go to GRAD
202320 on Sharepoint.
- You should see a folder under your name. If not, submit
via your CS account (see below).
- Inside that will be a Needs Action folder. Upload your document into that folder and then...
- Tag it for submission to the G10 category.

a SharePoint folder with your name, then you may, alternatively,
submit the form here.
Do not send your forms via email. Do not drop off paper
copies of your forms at the CS Department office.
